Job Description
Under general supervision provides therapeutic intervention and advanced social work services to patients and their families to enhance comprehensive, integrated and uninterrupted care across the care continuum and to have continuity of care in the community. Acts as a resource to Clinical Social Worker I incumbents. Demonstrates positive interpersonal relations with patients, families, visitors, and co-workers in accordance with UMMS and departmental policies and procedures.
General Summary
Autonomouslyfunctions as a member of the multidisciplinary healthcare team to provide case management, therapeutic crisis intervention, biopsychosocial assessment, therapy and counseling, referral, treatment planning, and other clinical social work services within their practice area. Maintains expert knowledge of available community resources to address the diverse needs of patients, groups, and families. Uses their understanding of the social determinants of health and clinical best practices to identify interventions for patients that enhance their functioning within their psychosocial environment.
