Location: Cincinnati, OH
Description: The Judge Group is seeking a Case Manager.

This job will have the following responsibilities:

• Responsible for health care management and coordination of Molina Healthcare members in order to achieve optimal clinical, financial and quality of life outcomes.
  
• Works with members to create and implement an integrated collaborative plan of care. Coordinates and monitors Molina members progress and services to ensure consistent cost effective care that complies with Molina policy and all state and federal regulations and guidelines.
  
  
• Develops and implements a case management plan in collaboration with the member, caregiver, physician and/or other appropriate healthcare professionals to address the member needs. o Performs ongoing monitoring of the plan of care to evaluate effectiveness.
  
  

Qualifications & Requirements:

• 0-2 years of clinical experience with Case Management experience.
  
• Candidates must have the following: Either a RN or LSW licensure
  
• They should have waiver case management experience
• They must have a vehicle and be licensed to drive
• Position is 100% remote, but candidates will be required to complete in home visits so they will need to reside in the Dayton, Ohio area. Montgomery or Greene County.
