Our client is currently seeking a CASE MANAGER RN to join their team!
Scope of role:
Location: Marysville, WA
Start & end times of shift: 7:30am-4pm M-F
Job Engagement: Contract
This job will have the following responsibilities:
- Hospital Transition calls for Medicare Advantage patients
- Care Coordination for complex, high-risk patients
- Motivational interviewing with patients and families
- Palliative Care conversations when appropriate
- Participation in Interdisciplinary Team meetings
- Washington RN license
- Ambulatory Care experience
- Outpatient Care Coordination experience
- Care Management skills
