Shift: Shift 1

Work schedule: Working 1 day/week or more to assist with additional workload and cover for PTO/vacations. 8-1630, No weekends/holidays

Job Summary:

Provides primary clinical care management and coordination of care for assigned patients under the direction and clinical supervision of the medical director or designee for the program. Services provided may include direct patient care according to scope of practice, coordination of program resources, coordination of quality improvement initiatives, and coordination of the continuum of care to these patient populations as well as to any patient in the clinic. Other duties as assigned.

EXPERIENCE DESCRIPTION:

Three to five years of experience in nursing field and hands on patient experience in an acute care setting is required. Procedural experience is preferred.

EDUCATION DESCRIPTION:

Professional knowledge of nursing theory and practice at a level normally acquired through the completion of a Nursing degree from an accredited School of Nursing is required. Bachelor's degree is preferred.

SPECIAL SKILLS DESCRIPTION:

Knowledgeable in the care of patients undergoing procedures.

LICENSURE DESCRIPTION:

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 3 months of hire. Advanced Cardiac Life Support for the Healthcare Provider (ACLS) or an AHA approved equivalent is preferred.
