Description: Nurse Practitioner - In-Home Wellness
Join our team! We're seeking a compassionate NP to conduct in-home assessments for Medicare beneficiaries in Jackson County, OR.
Your role:
* Conduct comprehensive health assessments
* Educate patients on health management
* Identify and address care gaps
* Collaborate with healthcare teams
