ABOUT THE JOB

The Mental Health Triage Coordinator is responsible for assessing patient needs, managing risk assessments, providing supportive counseling, and coordinating care plans within our mental health services. In addition, the Mental Health Triage Coordinator can assist with patient connection issues, managing referrals, connecting with additional resources, and work closely with the clinical team in helping each patient reach their optimal health and assuring a streamlined coordination of care.

ESSENTIAL DUTIES & RESPONSIBILITIES

Clinical Duties (may include, but not limited to):

  • Triage mental health calls assessing urgency and proving initial support and guidance
  • Conduct patient assessments and determine the appropriate level of care.
  • Manage and prioritize urgent mental health cases.
  • Collaborate with healthcare providers to develop and implement individualized care plans.
  • Collaboration with other mental health and medical clinicians for integrated care for patients

Administrative Duties (may include, but not limited to):
  • Using computer applications, preferably Microsoft Office suite
  • Timely completion of documentation within EMR
  • Work to address other social needs of patients and utilize appropriate external and/or internal referral sources for specialty care, hospitalization, or advanced psychiatric assessments and treatments

QUALIFICATIONS

Associate's or Bachelor's in Mental Health-related field of Social Work, Counseling, Psychology, or a related field. Master's Degree preferred. Graduate students currently pursuing relevant degrees may also be considered. 2 or more years of experience in mental health settings, such as hospitals, clinics, or community organizations is strongly preferred.
  • Required experience in crisis intervention, patient assessment, and case management.
  • Experience in triage, including assessing the urgency of patient needs and determining the appropriate level of care.
  • Preferred experience in providing mental health services, which may include evaluation, diagnosis, treatment of mental illness, and other psychological issues

  • Strong verbal and written communication skills for interacting with patients, families, and healthcare professionals.
  • Sensitivity and understanding of diverse cultural backgrounds and experiences.
  • Ability to manage multiple cases, prioritize tasks, and coordinate with different healthcare providers.
  • Ability to work within a multidisciplinary team

Pay Range: $60,000 - $80,000/yr

The actual offer may vary dependent upon geographic location and the candidate's years of experience and/or skill level.
