The position provides pharmaceutical care services to acute care (internal medicine, family medicine, intensive care, infectious disease) patients in a hospital setting. Specific areas of focus will include medication management of adult patients on the Hospitalist and Family Medicine services, providing clinical support, education, coordination of medication therapies, and monitoring related to medication therapy. It also serves as a preceptor, educator, and role-model practitioner for Doctor of Pharmacy students and pharmacy residents and carries out pharmacy practice responsibilities for the Department of Pharmaceutical Care. This is a 0.5 FTE (50%) position, with hours Monday-Friday during the first shift time frame, primarily.

Position Responsibilities

• Responsible for the operation of a hospital pharmacy that provides a full complement of drug therapy monitoring, drug dispensing and quality control services for all patient care units.

• Serve as pharmacist responsible for complete review of patients', medication therapy for appropriateness, which includes screening for drug-drug and drug-food interactions, drug-disease contraindications; adverse drug reaction detection and reporting; pharmacokinetic evaluation of serum drug levels and patient-specific laboratory data; communicating with medical staff to recommend drug therapy modifications (i.e., changes in drug, dose, route or timing of administration); implementing Pharmacy and Therapeutics Subcommittee approved criteria for safe and cost-effective medication use across the continuum of care.

• Respond to drug information questions form UIHC health care professionals.

• Assure adherence to all investigational drug studies dispensed for patient care.

• Ensure quality of the medication distribution system, including all drug products and informational materials issued by the Pharmacy.

• Ensure that all service transactions conducted by the pharmacy are fully authorized and properly documented as required by federal, state and hospital regulations.

• Responsible for selecting and adjusting medication doses when applicable, including dosage calculation, order writing, initiation of laboratory order requests, laboratory data interpretation, and facilitation of dosage changes.

• Maintain effective working relationships with other members of the healthcare team.

• Other duties as assigned.

SUPERVISION RECEIVED

Supervision is received from the Pharmacy Manager, a Pharmacy Supervisor, or other designated official.

SUPERVISION EXERCISED

Functional and administrative supervision is exercised over support staff and students.

QUALIFICATIONS

Education Requirements

  • Pharm. D. (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy and
  • Completion of an ASHP-accredited PGY1 pharmacy practice residency OR 2 years' health system pharmacy experience in an acute care environment with internal medicine experience.

Experience Requirements
  • Current licensure by the Iowa Board of Pharmacy Examiners is required.
  • Completion of an ASHP-accredited PGY1 pharmacy practice residency OR 2 years' health system pharmacy experience in an acute care environment with internal medicine experience.
  • Excellent written and verbal communication skills are required.
  • Job-related experience with and/or commitment to service excellence is required.
  • Professional experience working effectively with individuals from a variety of backgrounds and perspectives.
  • Ability to work independently, prioritize multiple stakeholders and tasks, manage details, and deliver results in an environment with competing demands.
  • Strong interpersonal skills and demonstrated ability to work with others.

Desired Qualifications
  • Additional training or certification (i.e., specialized residency and/or board certification in an area of practice) is desirable.
  • Satisfactory completion of an ASHP-accredited PGY2 residency is highly desirable.
  • Active board certification is highly desirable.
