Location: REMOTE
Salary: $38.00 USD Hourly - $40.00 USD Hourly
Description: We are currently seeking RN Case Managers to work REMOTELY for a fantastic client! If you have previous case management experience with the Medicare/Medicaid population specifically focusing on Special Needs Plan, this is the perfect opportunity for you. Please see details below.

• Can sit anywhere but must be able to work Mon-Fri, 8a-5p EST or CST
• Job Summary: The RN Case Manager plays a crucial role in enhancing the health outcomes of our members. Through the case management process, you will assist members who are facing the challenges of illness and injury.
  

Job Responsibilities:

• Evaluate individual member needs and develop personalized care plans to address their specific health concerns.
• Collaborate with healthcare providers, insurers, and other stakeholders to ensure seamless delivery of care.
• Track member progress and adjust care plans as needed to optimize outcomes.
• Communicate clearly and effectively with members, healthcare providers, and internal teams.
• Educate members about their health conditions and advocate for their needs.
• Work independently and effectively communicate with internal and external customers.
  
• Assess members' health status, identify treatment gaps or barriers, and create personalized case management plans that involve all relevant stakeholders.

Qualifications & Requirements:

• Active Compact State RN License
• At least 2 years current/recent Case Management experience
• Special Needs Plans (SNP) experience - HIGHLY preferred
• Plan of Care experience
• Bilingual - English/Spanish (HIGHLY preferred)
• Experience with senior population and chronic conditions.
