About the job:

We are looking for an experienced Social Services Assistant with a passion for geriatrics. Our Social Service team are key players in providing support and service to our residents and their families. Our careers offer opportunities to make a positive impact in the lives of seniors every day.

Essentials:
  • Coordinates resident transfers and discharges
  • Completes assessments of residents
  • Develops Care Plans and attends Interdisciplinary Care Conferences for residents
  • Initiates Reporting and documentation in the medical record
  • Develops and promotes the department through marketing efforts throughout the professional and social community
  • Serves on committees as required by Social Services Director
  • Serves as rights advocate liaison to the facility
  • Advises the Director of Social Services of major problems or concerns of residents or their family members
  • Coordinates resident admissions
  • Performs other duties or special assignments as directed by the Director of Social Services

Required Qualifications:
  • High school diploma or equivalent education required;
  • Licensed where applicable by the State; State requirements may dictate supervision and degree type
Preferred Qualifications:
  • 1 year of experience with LTC
  • Bachelor's degree in Social Work or related degree in social sciences
